Workers’ Compensation Process
All work-related injuries must be reported to your supervisor immediately and reported using the information listed below by following steps one through four. All injuries must be reported, even if you do not seek medical treatment.
IMPORTANT: Workers’ Compensation will cover all medical expenses and may provide temporary income benefits if eligible. Employees may use available paid leave for doctor visits and to cover extended absences due to their work-related injury. For questions please call Deanna Martinez at (830) 672-9551.
Gonzales ISD provides Workers’ Compensation benefits through the Texas Political Subdivision.
- Employee, supervisor, witness, or campus nurse may complete the First Report of Injury.
- Submit the form to Deanna Martinez at the Administration office within 10 days of injury.
- If a doctor’s appointment is needed, please call Deanna Martinez at ext. 1330. The Human Resources office will contact Sievers Clinic to inform them about the incident and schedule an appointment for the injured employee.
- Attend all follow-up doctor’s appointments and provide the Human Resources office with any additional information, including paperwork about your accident/injury.
- In the event of an emergency, call 911 or go to the nearest emergency room.
- Emergency room visits must then be reported to Deanna Martinez within 3 days. If a follow-up appointment is needed after an ER visit, it is the employee’s responsibility to schedule it with Sievers Clinic at (830) 672-8502.
